Provider Demographic Information


What demographic information do providers need to attest to?

Per CMS and the Consolidated Appropriations Act, practitioners are required quarterly to attest and update their demographic information. This may be accomplished either by confirming the location(s) where members are being treated or by updating information for accurate directory information.

Elements attested to quarterly:

  • Name
  • National provider identifier (NPI)
  • Tax ID number (TIN)
  • Clinic location address (street name, city, state, zip code)
  • Phone number
  • Email address
  • Website URL
  • Clinic name
  • Clinic TIN
  • Gender
  • Hospital affiliations
  • Clinic ADA accommodations in place
  • Office hours
  • If accepting new patients (per location)
  • If telehealth services offered
  • Specialties
  • Languages spoken
  • Cultural sensitivity/competency training completed
