The Select Health Quality Provider Program

Programs encompass primary care, women's health, mental health, and nephrology specialties, all currently available for Utah providers. For qualified primary care providers in Utah, Eastern Idaho, Nevada, and Colorado, we offer the Select Health Quality Plus Provider Program in conjunction with Risk Adjustment.

Primary Care

Primary care providers offer comprehensive health services, including preventive care, diagnosis, and treatment.

Women's Health

Women's health providers offer specialized care in areas like reproductive health, pregnancy, menopause, and more.

Mental Health

Mental health providers diagnose, treat, and support emotional, behavioral, and psychological issues.

Nephrology

Nephrology focuses on kidney health, treating diseases like chronic kidney disease and hypertension.

Endocrinology

Endocrinology studies hormones and glands, treating imbalances like diabetes and thyroid disorders.

Risk Adjustment

Risk adjustment predicts healthcare costs based on patients' health status to ensure fair provider payments.

How to Sign up for Reports:

To support clinics in their transformation to a patient-centered medical home care delivery model, Select Health provides clinics with enhanced reporting, a consultant resource, and the opportunity to earn quarterly performance payouts with an annual bonus structure. Signup can take up to 30 days to process.

Care Management

As part of Select Health member benefits, we maintain a trained care management team of licensed mental health providers and registered nurses who work closely with providers and patients to help better manage chronic or complex conditions, coordinate complex care, and ensure follow-up support. For Select Health members, there is no extra cost for care management services.
