About Care Management

As part of Select Health member benefits, we maintain a trained care management team of licensed mental health providers and registered nurses who work closely with providers and patients to help better manage chronic or complex conditions, coordinate complex care, and ensure follow-up support. For Select Health members, there is no extra cost for care management services.

If you have patients who have difficulty adhering to their plan of care, Select Health care managers can save you time by offering:

• Experienced nurses and behavioral health clinicians who can help identify barriers impacting your patient’s ability to adhere to their care plan
• Assistance with identifying in-network secondary providers and services and coordinating this care when needed (e.g., specialists, pain management services, Substance Use Disorder treatment)
• Coordinating access to social and other services in the community (e.g., transportation, housing, food, financial assistance, employment services)
• Helping members optimize their health plan benefits
• Coordinating access to mental health care

Healthy Beginnings

Healthy Beginnings is a specialized care management program for pregnant members included in their Select Health benefits.

The Healthy Beginnings program offers:

• Support and education to optimize health during pregnancy
• Potential incentives for completing prenatal and postnatal visits
• Assistance with breastfeeding, including information on how to obtain a free breast pump covered by the member’s health plan
• Assistance with claims and benefit questions
• Nurse care management support for high-risk pregnancies
