We are excited to partner with you to help Select Health members live the healthiest lives possible. We will work with you throughout the credentialing process for the Select Health networks.
Step 1.
Complete and send the Select Health Panel Request form to chris.engh@selecthealth.org.
Step 2.
The Select Health Provider Contracting Team reviews your request form, and if approved, we will contact you for more information and to discuss a contract and rates.
Step 3.
The Select Health Provider Contracting Team will present your contract and rates proposal to our committee for review and approval.
NOTE: You can request information on the status of your application. We will notify you of any issues that may be identified, such as discrepancies or other issues with the information you provided, and you have the right to correct erroneous information. You will be notified of any problems in obtaining the required verifications.
Step 1.
Select Health notifies you directly of credentialing decisions via email. During the review process, you may be contacted by a Provider Contracting Rep to discuss rates. If rates are approved, you will receive a Participating Facility Services Agreement (PFSA).
Step 2.
Review, sign, and send the PFSA to us. Select Health will execute the Agreement and send you a Welcome Letter that includes links to provider online resources and how to contact your facility/vendor representative.
Step 3.
Verify that your information is correct in our online Facility Directory. Your listing should appear as of the effective date of your contract. Should there be an error in your listing, please advise us by emailing chris.engh@selecthealth.org.
