Clinician Credentialing & Contracting

For credentialing, Select Health uses the Council for Affordable Quality Healthcare (CAQH) and CertifyOS to collect and verify provider information. We will work with you throughout the credentialing process for Select Health networks.

Utah Credentialing and Contracting Process

For networks outside of Utah

For networks in Colorado, Idaho, and Nevada, a Participating Provider Services Agreement (PPSA) must be signed by both parties prior to credentialing. Once credentialing is approved, you will be notified and effective dates will be shared. 

Getting credentialed

Step 1: Submit the Join Our Panel form

  • If you haven't already, obtain a CAQH ID number by registering at CAQH and completing your profile.
  • Next, complete and submit the Join Our Panel online request form. 

NOTE: An active license in the state(s) where you practice is required for credentialing and network participation.

Don't have a CAQH ID number?

Complete your registration through the CAQH portal (or the ADA online portal for dental) to obtain your login and receive your CAQH ID.

Step 2: Select Health review (2-3 weeks)

Next, your Select Health representative will review your submitted request to confirm it's ready to move forward. This process takes 2-3 weeks.

Step 3: CertifyOS verification (2 weeks)

Once approved, the application will be sent to CertifyOS for primary‑source verification. This process generally takes 2 weeks.

Step 4: Credentialing Committee and network approval (2-4 weeks)

From here, your application will go to the Credentialing Committee and Practitioner Panel Strategy Committee for network approval. We will notify you of any issues, such as discrepancies or other issues with the information you provided. This takes between 2-4 weeks.

Step 5: Provider receives and signs PPSA

Next, Select Health will send you the Participating Providers Services Agreement to sign and return to us. This ensures you're a contracted provider or clinic with Select Health.

Step 6: Select Health signs and returns PPSA

Once you've signed the PPSA, Select Health will countersign and provide you with a fully executed copy.

Step 7: Welcome email

Once credentialing is finalized, you will receive a welcome email with links to provider online resources and contact information for your provider relations representative.

Step 8: You're enrolled in Select Health systems

Finally, allow up to 4 weeks for your listing to appear in our online provider directory. Verify that your information is correct and contact your provider representative if you notice any issues.

The credentialing life cycle

Credentialing is a continuous process. You'll need to recredential every 3 years to avoid termination. Providers who wish to end their relationship with Select Health should notify their provider relations representative so records can be updated and credentialing communications stop.

Initial credentialing
Verifies education, training, licensure, work history, and sanctions.
Ongoing monitoring
Tracks licenses, certifications, malpractice coverage, and sanctions. Maintain up-to-date documentation and review credentials proactively.
Recredentialing
All providers MUST recredential every 3 years. Keep your CAQH profile updated, especially your email address, to simplify recredentialing.

Frequently asked questions

Even with automated and streamlined processes, credentialing can still be delayed. Common causes include:

  • Outdated CAQH profiles
  • Missing attestations
  • Expired documents (malpractice insurance, licenses)
  • Missing or inaccurate board certifications
  • Missing explanations for disclosure responses
  • Incorrect or outdated email addresses

Please keep in mind that credentialing is just one part of the onboarding process. After credentialing is completed, additional steps (such as network committee approvals, contracting, and system setup) take place separately and follow their own timelines.

You have the right to review your credentialing application, including any information obtained from any outside source, except for references, recommendations, or other peer reviewed-protected information.  

We will notify you of any issues that may be identified, such as discrepancies, problems obtaining the required verifications, or other issues with the information you provided, and you have the right to correct erroneous information.

Select Health works with CAQH and CertifyOS to collect and verify provider information at various points in the credentialing process. Each play an important role in getting providers credentialed. 

CAQH: CAQH is a national repository where providers maintain their demographic and professional information. Select Health and CertifyOS both rely on the CAQH application, so keeping your CAQH profile current and attested is essential. You may receive attestation reminder emails from CAQH.

CertifyOS: CertifyOS peforms primary-source verification using the information in your CAQH application, along with other data sources. They validate licenses, certifications, and required documents to ensure accuracy. You may receive emails from CertifyOS on credentialing/recredentialing notices, requests for missing information, or personal statement requests.

Select Health: Select Health is the final decision maker. We review the verified information from CertifyOS, apply our credentialing criteria, and present cases to the Credentialing Committee. Emails you may receive from Select Health include credentialing approvals, requests for additional information, incomplete application notices, and recredentialing termination notices for non-response.
