Cancel your plan

To terminate your plan follow the instructions for your scenario below: 

Individual and Family plans purchased through Select Health

To cancel your coverage, complete the Individual Plan Change Form and send it to Select Health.

Important: We must receive your form before the first day of the month you want your coverage to end.

Idaho Individual and Family plans purchased through Your Health Idaho

To cancel your coverage, contact Your Health Idaho directly.

Important:

  • Cancellation requests must be submitted at least 14 days before your requested termination date.
  • Any unpaid premium balances must be paid before you can enroll in a Select Health plan again.

If your plan shows as terminated, you may qualify for reinstatement.

Call Select Health at 844-442-4106 to discuss your options.

Retroactive claim denials

A retroactive denial happens when a claim that was previously paid is later denied. If this occurs, you may be responsible for paying for the services received.

This can happen for reasons such as:

  • A claim was paid during the second or third month of a grace period, but premiums were not paid in full.
  • A claim was paid for services received when you were not eligible for coverage.

How to Help Avoid a Retroactive Denial

You can reduce the risk of a retroactive denial by:

  • Paying your premiums in full and on time.
  • Checking that the services you receive are covered by your health plan.
  • Using providers in your plan's network whenever possible.
  • Asking your provider if a service requires authorization or has coverage limitations before receiving care.
