2. Select the Health & Wellness Rewards box in the View All Tasks section
3. Select the Track Gym Activity button
4. Enter your monthly Gym activity
Follow these steps to earn Physical Activity Rewards.
1. Login to your Select Health account
2. Select the Health & Wellness Rewards box in the View All Tasks section
3. Select the Track Steps or Enroll button
4. Sign up for a Personify Health account (formerly Virgin Pulse) or select Home then Rewards
5. Enter your activity in the Take 7,000 steps in 20 days section

2. From Tasks select “Redeem My Reward Points”
3. Select “Redeem Now” linking to the Rewards homepage
4. From the Rewards homepage select “Gym/Physical Activity”
5. Enter the Pledge Date, Gym or Physical Activity, and Rate Us feedback.
Your account will then be credited 20 points.
Eligible members in Utah, Nevada, and Idaho can earn up to $240 per person, per year or $580 per family, per year. Amounts received may be considered income and subject to tax.

The Utah/Idaho Terms and Conditions apply to Select Health members in Utah and Idaho on Small Employer or Individual and Family plans. This includes members in Idaho on some Large Employer plans.
Loyalty/Signature Network Terms and Conditions apply to Select Health Individual/Family plan members in Nevada and Colorado, or Utah Individual/Family plan members with the Signature Provider Network.
All completed activities eligible for reward points/dollars must be entered and redeemed online through your Select Health member account by November 30. This includes gym information and activity tracking data submitted to Virgin Pulse.
This is the last day during the year that you can redeem reward points for your choice of gift card(s), This date will always fall on the same date designated as the Activity Completion deadline.
Reward points accumulate January through December, resetting yearly in January. You can redeem your points monthly or save them and redeem them when desired for gift card(s) of greater value.
If you have unredeemed reward points at the end of a calendar year, you'll be sent Visa gift card(s) totaling their value.
Select Health Medicare members are eligible for the Wellness Your Way Reimbursement benefit or the Healthy Living program. For details visit, Medicare Wellness.
Select Health FEHB members are eligible for the FEHB Healthy Living program. If you have specific questions about FEHB benefits, call Member Services at 844-345-3342.
Although not required, we strongly encourage all eligible members to take advantage of the complimentary Personify Health (formerly Virgin Pulse) Online Health Check.
To activate your Wellness Rewards Visa card, enter the 16-digit number found on your Visa card with your access code. Your access code is the last four digits of your Select Health subscriber ID number located on the upper right-hand side of your insurance card.
The correct phone number to activate your Visa card is 1-833-848-5768.
The Activity Completion and Reward Redemption deadlines are November 30, 2025.
