What is Care Management?

Select Health Care Managers can help you navigate your health plan and care choices. They support our members’ mental, physical, and emotional health needs so they can live their healthiest lives possible. Care Management is completely voluntary and confidential. It’s a program included in most Select Health plans with no added cost.

How does Care Management work?

First, you will be paired with a dedicated Care Manager from our team of experienced healthcare professionals—including nurses, social workers, and care navigators. Medicaid members also have access to community health workers.

From there, we’ll get to know you and your health needs, and recommend care options that fit your situation. We’ll work around your schedule and can meet with you on the phone, through video chat, or by email.

What are the benefits of Care Management?

Care Management can lead to better health and reduced costs through regular preventive care and support with chronic conditions like asthma, diabetes, or heart disease. Care Managers are advocates for you and your health—a trustworthy source of information. Here are examples of how we can help you:

Care managers can help you find resources like food, housing, and transportation if needed.

Care Mangers can contact and coordinate with your doctors so that you have access to the care you need.

Stay on top of your health.

Care Managers can help you to take advantage of preventive care visits, immunizations, and recommended screenings.

Suggest healthy activites.

Care Managers can help find health and wellness activities, educational materials, and community resources.

Care Management Support & Services

You can have great health insurance benefits—but if you aren’t sure how to use them or where to go to get the right care, it can be frustrating. Fortunately, your plan comes with access to Select Health care managers who are happy to be your guide to all things healthcare. They will listen to your concerns, support you in finding the right care, help you understand how to manage tough health conditions, and be a trustworthy source of information.

If you’ve never used a care manager before, think of them as someone who will be on your side, like a part of your care team. Here are just a few of the things a care manager can do:
