Select Health providers in Utah can help their young patients take an important step toward lifelong health by sharing Intermountain Health’s Growing Your Health program. This program offers free resources designed to help teens and young adults build the knowledge and confidence they need to manage their own healthcare.
What the program offers
The Growing Your Health program helps young people and their families prepare for adult care with practical education, tools, and activities. Resources include support with:
- Advocating for their care and setting healthcare goals
- Finding adult healthcare providers
- Navigating insurance and the healthcare system

Upcoming event
In addition to the online resources available through the program, young adult/teen patients can learn more by attending the 7th Annual Teen to Adult Healthcare Transition Summit, hosted by Intermountain Health. There is no fee to attend the summit, and both virtual and in-person attendance options will be available.
