Select Health dental ID cards are designed to help providers quickly identify a member’s dental network and key benefit information. Because ID cards vary by plan type and network, dental offices should review the card carefully before providing services and verify benefits when needed.
What dental providers can find on the ID card
Select Health dental ID cards include details that can help your office confirm important information at the point of care, including:
- Member and subscriber information
- Dental network name
- Member ID number
- Claims mailing information
- In-network dental benefit details, when applicable
A member’s ID card may also cover enrolled dependents, so providers should confirm eligibility and benefits for the individual receiving care.
Commercial dental networks
Select Health commercial dental ID cards may identify one of several dental networks, including Classic, Prime, or Fundamental. Network availability can vary by county, so it’s important to confirm your practice participates in the member’s specific network before services are provided.
Individual and Small Employer plans may include embedded preventive pediatric dental benefits on the Classic network for services such as cleanings, fluoride applications, oral exams, sealants, and x-rays.
Medicare dental benefits
Select Health Medicare dental plans may include preventive, basic, and major dental benefits embedded in the member’s medical coverage. Medicare dental ID cards include dental plan information, in-network benefit details, and instructions for members to present the card before receiving dental services.
Because benefits can vary by plan, providers should review the member’s Evidence of Coverage and contact Member Services to verify coverage for services not listed on the card.
Reminder: Verify benefits before providing care
ID cards provide helpful benefit and network information, but they are not a guarantee of coverage. To help avoid confusion for patients and your office, verify eligibility, benefits, and network participation before services are rendered. This is best accomplished by logging into the provider benefit tool.
