Pharmacy tiers of coverage

Prescription coverage for members is usually divided into four or five tiers, depending on the plan type. Each drug is covered under a specific tier, and each one corresponds to a copay or coinsurance amount.

Tier 1
Lowest cost (mostly preferred generic drugs)
Tier 2
Lower cost (mostly non-preferred generic drugs)
Tier 3
Higher cost (mostly preferred brand-name drugs)
Tier 4
Highest cost (mostly brand-name drugs
Tier 5
Injectable drugs and specialty medications

NOTE: Not all plans have a fourth or fifth tier. Some plans cover injectable drugs and specialty medication under medical benefits.

Generic drugs typically cost less than brand-name drugs and have lower copays/coinsurance. They have the same active ingredients, dosages, quality, safety, and strength as their brand-name counterparts.

Intermountain Home Delivery

Intermountain Home Delivery Pharmacy is the mail order contract pharmacy for various group plans with Select Health.

As patients enroll in our service, physicians may receive a request for new prescriptions to be sent to the Intermountain Home Delivery Pharmacy. The processing and delivery time for prescriptions is five to seven days, offering a convenient way to provide medications.

Physicians can submit prescriptions via:

  • E-prescribe: Send to “Intermountain Home Delivery Pharmacy"
  • Fax: 801-442-4179
  • Telephone: 855-779-3960

Medications that can be delivered:

  • Intermountain Home Delivery Pharmacy may be used to fill maintenance medications. The pharmacy will fill a 90-day supply at a time. 

Medications that cannot be delivered: 

  • Acute-type medications and controlled substances may not be filled using Intermountain Home Delivery Pharmacy. Those prescriptions can be sent to any community pharmacy.
