Individual Agent of Record (AOR) form

  • Not every scenario calls for a signed AOR form. If you are acquiring business from another agent or you have purchased a portion/all of their business, send an email to Kenzie.gray@selecthealth.org and include both agents. We will process those transfers with the following effective month.
  • Individual AORs must be received by January 15, 2024, for a January 1, effective date.
  • AOR letters must be signed by both the member and the agent within 30 days of submission. AOR letters will be returned and not processed if signatures are missing, or the date on the letter is more than 30 days out.

Small Employer new groups

Please remember that ALL full-time employees need to be listed on the census, even employees waiving coverage. For groups that are being submitted by December 15, and would like the usual participation and contribution requirements waived, the following is still required:

  • They must be an eligible group with at least two full-time employees.
  • All full-time eligible employees are accounted for and included on the census as either enrolling in or waiving coverage.
  • Groups not submitted in Select Health Link by December 15 must meet the usual contribution and participation requirements, without exception.

Mineral® Human Resources (HR) and Compliance Services

NEW in 2024 for Small Employer groups in Utah and Nevada.

We are partnering with Mineral to provide HR and compliance services to our Small Employer groups in Utah and Nevada. This service will work much like our built-in Intermountain Employee Assistance Program (IEAP). Select Health is contracting with Mineral to cover the cost of the service and groups will contact Mineral directly for help. There will be no system connections. Select Health will only provide Small Employer group details to allow for employer access, not employee information. Employers can choose whether to use the services.

Please remind your groups that a Mineral team member will be reaching out to them after January 1 to schedule a demonstration.

Mineral HR Experts: HR certification through either/both: HR Certification Institute (HRCI); Professional HR (PHR) or Senior Professional HR (SPHR) Society for Human Resources (SHRM); SHRM-Certified Professional (SHRM-CP) or SHRM-Senior Certified Professional (SHRM-SCP).

Mineral Benefits Experts (Health, Welfare, and Retirement): International Foundation of Employee Benefit Plans (IFEBP): Benefit Experts are required to have their GBA (Group Benefits Associate); some also have their RPA (Retirement Plan Associate). When the GBA and RPA are combined, team members have their full Certified Employee Benefits Specialist (CEBS).
