Nevada EyePA manages the contracting process for optometrists and ophthalmologists in Southern Nevada looking to participate in the Select Health Nevada networks. The Nevada EyePA team will review submissions and assist providers throughout the onboarding process.
Getting started
To be considered for participation, please submit the following directly to Nevada EyePA:
- Doctor's cover letter
- Doctor's resume
- Letter of intent
- Full name
- Location
- Hours of operation
Nevada EyePA will respond within 5 business days to confirm the receipt of the request or to ask for additional information.
NOTE: Submission of an application or Letter of Intent and supporting documentation does not guarantee participation in the Select Health provider network. All applications are subject to network needs, credentialing requirements, and contracting review.
