Small Employer / Individual

Level Funded Plans for Utah Small Employer Groups.

We are now selling Level Funded plans for Utah Small Employer groups who have at least 15 enrolled employees. 

Simple Quoting via Select Health Link.

Agents can easily quote Level Funded plans using Link.

  • Need training? Contact your Account Manager to schedule a Level Funded training session for your agency.

Accumulator Options.

Agents can choose between plan year or calendar year accumulators during the quoting process.

  • If a group selects calendar year accumulators, we will credit accumulators accordingly.

Commission Details.

  • Default commission is $40 PEPM.
  • Agents can select a specific commission rate lower than $40 PEPM in $5 increments.
  • Bonus Alert: Earn a $75 PEPM one-time bonus for any new Level Funded business written in 2025.

Rx Formulary.

  • The formulary for these plans is Rx Select.

Provider Networks.

  • Available networks include Select Health Med and Select Health Value.

Large Employer Level Funded Wellness Rewards Program.

With Level Funding plans, Wellness Rewards programs and services are included as part of the administrative rate. This makes it easy to offer clients a comprehensive benefit that supports retention, satisfaction, and overall workplace performance.

Wellness Rewards programs and services included with level funding plans:

Digital Wellness Platforms.

  • Personify Health™: Track steps, complete health assessments, access digital coaching, and more.
  • Sworkit Health®: On-demand workouts—anytime, anywhere.
  • Augeo®: Optional platform for tracking and fulfilling wellness rewards.

Wellness Rewards Options.

Gym Membership Rewards: Earn points for gym visits, yoga, Tai Chi, or acupuncture.
Physical Activity Rewards: Earn points for completing 20+ days of 7,000 steps or equivalent exercise per *Note: Members can switch between programs monthly but can only earn rewards from one per month.*

Tobacco Cessation.

  • Quit for Life®: Personalized coaching and nicotine replacement therapy.

Health Education & Events.

  • Wellness Presentations: Two on-site or virtual sessions per year.
  • Health Fairs: Annual on-site events with screenings, assessments, and resources.
  • Flu Shot Clinics: Free on-site flu shots for members and dependents (18+).

Earning & Redeeming Rewards.

  • Eligible members (18+) can earn up to $240 per person or $580 per family annually.
  • Gym Membership Rewards are tracked via the Select Health member portal (not the app), and Physical Activity Rewards are tracked via Personify Health. 
  • Redeem monthly or annually via the Select Health member portal for gift cards or receive a prepaid Visa card if unredeemed by year-end.
