Participation Requirements

Clinics that participate in the Quality Provider Program strive to meet annual clinical goal thresholds as well as a series of participation requirements (outlined at an orientation meeting), such as:

• Educating patients on how to engage in their own healthcare plan
• Screening patients for social determinants of health
• Utilizing community resources
• Following up with patients after discharge from an inpatient setting
