Every year, Medicare evaluates plans based on a 5-Star Rating System. Select Health Medicare HMO plans received 4 out of 5 Stars and Select Health Medicare PPO plans received 3.5 out of 5 Stars for the contract year 2025. Visit medicare.gov for more information.

The benefits mentioned are part of a special supplemental program for chronically ill enrollees. Eligible chronic conditions include diabetes, hypertension, musculoskeletal disorders, lung disorders, and cancer, as well as other conditions not listed. Eligibility for the benefits is not based solely on your condition and all eligibility requirements must be met before the benefits are provided.  For details, please contact the Plan.

The lower costs advertised in our plan materials for these pharmacies may not be available at the pharmacy you use. Out-of-network/non-contracted providers are under no obligation to treat Plan members, except in emergency situations. Please call our customer service number or see your Evidence of Coverage for more information, including the cost-sharing that applies to out-of-network services.
