Small Employer
Mineral
Make ERISA compliance easier for your clients
January 1 renewals are a good time to remind employer clients to review their required ERISA Wrap Plan Documents and Summary Plan Descriptions (SPDs). Keeping these documents current can help groups avoid Department of Labor audit penalties and unnecessary legal expenses.
An included resource that adds value
Through Select Health’s collaboration with Mineral, Fully Insured and Level Funded employer groups have access to the Benefits Document Creator. This included tool helps clients meet compliance obligations without paying third-party legal fees.
- No additional cost: Create customized, legally compliant Wrap SPDs as part of the group’s plan.
- Faster documentation: A guided process helps clients complete documents in minutes instead of waiting weeks for outside drafts.
- Ongoing support: Mineral monitors its templates against changing federal ERISA and ACA guidelines.
- More value at renewal: Sharing this resource gives you another practical way to support clients during renewal season.
Share it with your clients. Watch this two-minute overview video to see how the tool works. Consider including the video in upcoming client communications or mentioning the Benefits Document Creator during renewal conversations.
Preparing for Open Enrollment: submit complete group applications to avoid delays
As we prepare for another busy Open Enrollment season, we want to help make the group submission process as smooth as possible. One of the most common causes of processing delays is incomplete group paperwork. Taking a few extra minutes to ensure all required documentation is included when you submit a group can help avoid the need for additional follow-up and keep your requested effective dates on track.
Important reminder about processing times
Open Enrollment is our busiest time of year, and every group submission is reviewed and processed manually. During this period, we ask for your patience as our teams work diligently to review each application as quickly and accurately as possible.
To help us process your groups efficiently, please submit complete applications as early as possible and allow additional time for review. Missing documentation often results in follow-up requests and can extend processing timelines.
New small group submission checklist
Before submitting a new small group, please verify the following items have been completed:
Group documentation
Submit documentation for every eligible employee and owner, whether they are enrolling or waiving coverage. Required documentation includes:
- Employer State Unemployment Tax Form or Wage and Tax Form.
- If the employer has been in business for three months or less, provide the most recent two weeks of detailed payroll records for all eligible employees.
- If owners or partners are not listed on the State Unemployment Tax Form, provide one of the following:
- Articles of Incorporation or Organization (preferred)
- Stock Certificate
- Operating Agreement
Please note that additional documentation may be requested in certain situations.
Employee forms
For employees enrolling in coverage:
- Documentation is still required for employees who are eligible for coverage but choose not to enroll.
- A completed employee application and Supplemental Form must be uploaded through Select Health Link for each eligible employee.
- As an alternative, agents may submit an enrollment spreadsheet in Link containing all required enrollment information for eligible employees.
- For Nevada groups, the supplemental information is included within the employee application and does not require a separate form.
- Utah and Nevada groups must complete Page 3 of the Employee Application for each eligible employee waiving coverage.
- Idaho groups must complete Sections 1-3 of the Idaho Employee Application for each eligible employee waiving coverage.
- Waiver information helps verify eligibility requirements and ensures the group meets participation guidelines.
Payment and Sign & Pay requirements
Please ensure the following items are completed:
- Signed Group Medical Application (GMA)
- Group Dental Application, if applicable
- Group Eyewear Application, if applicable
- E-Check or Preauthorized Banking Form for future premium payments (preferred)
- Image of the check for the first month’s premium
The GMA can be completed through the Sign & Pay process in Link or submitted on paper.
Timeline reminders
All paperwork should be submitted in Link by the last day of the month prior to the requested effective date. Once a group enters review, any additional requested documentation must be submitted within 10 days following the effective date. If required items are not received by the deadlines, the effective date may need to be moved to the next available date.
If you have any questions, please contact your account manager.
Reminder: new business submission deadlines for ACA, Association, and Level Funded groups
As we prepare for the upcoming Open Enrollment season, we want to remind agents of the updated submission timelines for ACA, Association, and Level Funded for new group business. Following these deadlines will help ensure a smooth review process and allow coverage to become effective as requested.
Small Employer ACA group timeline
To ensure your client’s requested effective date can be met, please follow these important guidelines:
Initial submission requirements
All the required enrollment and employer paperwork must be submitted in Link no later than the last day of the month prior to the requested effective date.
Additional documentation period
Once the application has been reviewed, groups will have up to 10 days after the effective date to provide any additional requested items needed for final approval.
Missed deadlines
If required information is not received within the allotted timeframe, the group’s coverage effective date will be moved to the next available effective date.
Association and Level Funded group timeline
To ensure your client’s requested effective date can be met, please follow these important guidelines:
Submission requirements
For Association and Level Funded groups, all required enrollment and employer information must be completed and submitted through Link. All required contracts must be signed and payment must be made prior to the requested effective date.
Applications that are not completed in Link, including signed contracts and required payment, prior to the effective date may not be eligible for the requested effective date.
Why timely submission matters
Submitting complete and accurate paperwork by the deadline helps prevent delays in processing, ensures coverage begins as expected, and reduces the need for last-minute requests. We encourage agents to review all required documents before submission and work with their clients early to gather any missing information.

Large Employer
Help employers save on surgical costs
Ambulatory surgical centers (ASCs) provide high-quality, same-day outpatient procedures in a setting designed for convenience, efficiency, and affordability. ASCs offer a cost-effective alternative to traditional hospital outpatient departments for many common procedures while maintaining a strong focus on quality and patient experience.
How ASCs can lower costs
Many outpatient procedures can be performed in an ASC rather than in a hospital setting. Because ASCs are designed specifically for scheduled outpatient care, they often have lower facility costs, which can translate into lower out-of-pocket expenses for members.
For employers, encouraging employees to compare care settings can support more informed healthcare decisions and help maximize the value of their health benefits.
Additional benefits members appreciate
ASCs are also designed to create a more seamless experience for patients, including:
- Shorter wait times and more efficient visits
- Convenient same-day outpatient care (with locations across Colorado, Idaho, Nevada, and Utah)
- Smaller, easier-to-navigate facilities
- A more predictable and comfortable care experience
- Access to numerous locations across Select Health networks
These features can help reduce the stress often associated with surgical procedures while making it easier for members to receive the care they need.
A valuable enrollment conversation
By educating groups about ASCs and encouraging members to explore their options before scheduling outpatient procedures, agents can help employers promote smarter healthcare decisions, improve the member experience, and potentially reduce healthcare spending.
Select Health members can learn more about available care options and participating facilities through the provider search tool at selecthealth.org/find-care.
