Adding Dependents as a Result of Adoption

Placement papers and change forms should be submitted within 31 days of the child’s placement if the member intends to enroll the newly adopted child on his or her policy. The member must be enrolled in the plan and eligible for benefits on the date the child is placed for adoption with the member.

Adoption Indemnity Benefit

The adoption indemnity benefit was created as a result of Utah state legislation. Members are eligible for the adoption indemnity benefit when adopting a newborn that is placed with the member within 90 days of the child’s birth. Members have one year from the date of placement to submit a claim.

The member adopting the child must be eligible for benefits on the date the child is placed for adoption with the member. The child does not have to be enrolled in order for the member to receive the adoption indemnity benefit.

Eligible members on Utah contracts will be reimbursed at a set amount determined by state law.

Birth control is any method used to prevent pregnancy. There are many different kinds of birth control, and learning about the methods will help you find the one that is right for you.

Eye Exams and Care

Vision exams must be given by a participating doctor to be eligible for the highest level of benefits. Benefits for services preformed by nonparticipating doctors will be reduced or denied according to regular plan guidelines.

Infertility

Infertility is the inability to become pregnant after at least one year of unprotected intercourse. Sterility is the permanent inability to become pregnant because of a congenital anomaly, disease, or operation.

Intrauterine Device (IUD)

An IUD is a long-term birth control method. It is a small, T-shaped plastic device that is wrapped in copper or contains hormones.
