SelectHealth's disease management programs focus on intervention and management of existing chronic health conditions in the following areas:

Allergies & Rhinitis

The Allergies and Rhinitis program provides the following if you have SelectHealth pharmacy benefits:

  • Educational materials mailed directly to your home
  • Access to the Pharmacy Help Desk, which provides trained staff to answer questions on allergy and rhinitis treatment

Asthma

The Asthma Management program provides the following:

  • A quarterly newsletter including advice on managing your chronic asthma
  • Classes and access to a care manager to help you learn how to control asthma
  • Follow-up care if you have recently received treatment in the emergency room

Care Management

Care managers are specially trained registered nurses who assist patients with long-term chronic diseases. They help patients navigate through the healthcare system and follow doctor's recommendations for therapy and medications.

A SelectHealth representative often contacts members immediately following certain diagnoses to see if they would like the assistance of a care manager. However, we encourage members to contact us if they have questions or feel that someone they know would benefit from these services.

SelectHealth offers care management services to most members, including those with chronic medical conditions Care managers work with the member's doctor to develop a care plan based on immediate and ongoing needs. They listen to concerns and partner with providers, pharmacies, and hospitals to coordinate the best possible outcome

Cholesterol

The Cholesterol Management program provides the following if you have high cholesterol:

  • An educational video and resource materials to help you learn how to manage cholesterol
  • A follow-up letter and phone calls reminding you to get tested

Congestive Heart Failure

The Congestive Heart Failure program provides the following:

  • Access to a care manager who can answer your questions and resolve your concerns
  • Phone calls to help you learn about your medication, activity, or dietary needs
  • Brochures about congestive heart failure mailed to your home
  • Free classes to help you learn how to manage heart failure

Depression

SelectHealth helps you manage your depression. The Depression Management program provides the following:

  • Online educational materials and brochures about understanding and managing depression
  • Free depression screening tool
  • Access to a care manager who can answer your questions and resolve your concerns
  • Behavioral Health Advocates to assist you in finding a provider to best meet your needs
  • Antidepressant prescription refill reminder phone calls

Diabetes

The Diabetes Management program helps those of you with diabetes learn how to manage your disease and decrease complications. The program provides the following:

  • A quarterly newsletter to help you manage diabetes
  • Access to a care manager who can provide information and assistance
  • Reminders and incentives to get all your preventive care checkups
  • Free monitoring equipment (one time only)
  • Classes on diabetes

Hepatitis C

SelectHealth's Hepatitis C Management program provides the following if you have hepatitis C infection:

  • Advice on how to manage side effects of the treatment
  • A starter kit

Hypertension

SelectHealth helps you decrease your risk for complications by educating you on how to manage hypertension. The Hypertension Management program provides the following:

  • An educational DVD and kit to help you learn how to manage your blood pressure
  • Educational materials to help you manage your condition according to the latest clinical guidelines
  • Access to a care manager who can answer questions and provide information

Oncology

If you are diagnosed with cancer, SelectHealth's care managers work with providers, purchasers, patients, and families to determine probable costs, confirm benefit coverage, and expedite provider-recommended tests and treatment. Specifically, the managers help do the following:

  • Coordinate specialty care related to your condition
  • Understand your disease process and provide answers to clinical questions
  • Understand the policies relative to your condition and treatment
  • Understand the billing process
  • Work with SelectHealth to get bills paid promptly and correctly

SelectHealth's Migraine Management program offers you the following if you have pharmacy benefits:
