Applying for a New Plan

Applications for a new individual plan are accepted at any time. If approved, coverage will be effective on the 1st or 16th of the month, as determined by SelectHealth.

  • Singles - The applicant must be age 19 or older.
  • Married Couples without Children – The primary applicant must be age 19 or older. His or her spouse must be included on the application.*
  • Families with Children – The primary applicant must be a parent, age 19 or older. All family members, including spouse (if applicable) and children ages 18 and younger, must be included on the application.* Parents may also include dependent children ages 19 to 26 on the application, but are not required to do so.

*Exceptions may apply, such as when a child or spouse is covered by certain government insurance programs. Contact your SelectHealth-appointed insurance broker or the SelectHealth Individual Sales department for details.

Adding Dependents Ages 18 and Younger to an Existing Plan 

Dependents ages 18 and younger may be added during either of SelectHealth’s enrollment periods. These dependents are guaranteed coverage on a subscriber’s policy regardless of health status. Adding a dependent may result in an increase in premium.

  • For January 1, 2011 effective dates, the enrollment period is November 1, 2010 – January 31, 2011
  • For July 1, 2011 effective dates, the enrollment period is May 1, 2011 – June 15, 2011

Adding Dependents Ages 19 and Older to an Existing Plan

The subscriber may request to add dependents ages 19 and older at any time. These dependents are subject to underwriting and must qualify for coverage. Their coverage will be effective on the 1st of the month, as determined by SelectHealth.

Adding a Newborn or Adopted Child to an Existing Plan

Dependent enrollment may be allowed outside of the annual enrollment period for certain events, such as the birth or adoption of a child.
