Deductibles and Out-of-Pocket Maximums
These plans have four different medical deductible options. Each one has a corresponding medical out-of-pocket maximum, pharmacy deductible, and pharmacy out-of-pocket maximum. The deductible you choose will apply to all of your employees.

Copay Amount
There are two office visit copay options. Use our Benefit Summary to help with your selection.

Optional Add-ons
You can upgrade benefits for your employees.
  • $7,500 per pregnancy maternity deductible, if you have less than 15 employees (may lower premium).
  • Waiver of Medical Deductible for Office Visits
  • Pharmacy Deductible Waiver (only available if you select the Waiver of Medical Deductible for Office Visits)
