Important Plan Features
Networks
Each employee can choose from our Select Value, Select Med, and Select Care provider networks. It is your responsibility to let your employees know if you don’t want them to choose different networks.
Deductible and Out-of-Pocket Maximum
We offer four deductible options: three 80/20 coinsurance plans and one 100 percent coinsurance plan.
Our 100 percent coinsurance option has the highest deductible, but all covered services are paid at 100 percent after the deductible is met.
Employees only insuring themselves will have a single deductible. Employees insuring themselves and at least one other person must meet the family deductible before any benefits will be paid.
